A whistleblower's brave action has led to a crucial inspection at a worksite in Ubi, Singapore. The Ministry of Manpower (MOM) received disturbing videos revealing unsafe forklift practices, prompting an immediate response.
In one video, a worker stood precariously on the edge of a tall trash bin, a dangerous move that could have easily led to a fall and serious injury. Another clip showed a worker hopping onto the back of a moving forklift, a clear violation of safety protocols.
The MOM's swift action is a stark reminder of the importance of workplace safety and the potential consequences of negligence.
On November 21st, MOM officers conducted a surprise inspection at 50 Ubi Avenue 3, where they encountered a forklift operator wearing the same uniform as the workers in the videos. Despite the operator's denial, the officers had received feedback indicating otherwise, and they proceeded to investigate further.
But here's where it gets controversial... The inspection uncovered additional potential safety breaches. The warehouse's mezzanine storage platform, designed for loading and unloading goods, lacked proper fall prevention measures. A travel restraint system, which ensures workers are securely anchored while working near edges, was notably absent.
Furthermore, an unused table saw machine was found without its blade guard, exposing a dangerous blade that could cause severe injuries, even amputation.
MOM officers spent an hour interviewing workers and employers, assessing safety briefings, and investigating these potential hazards.
Mr. Sebastian Tan, director of MOM's Occupational Safety and Health Inspectorate, emphasized the seriousness of the situation: "The videos revealed unacceptable practices, including dangerous forklift operation and unsafe work-at-height practices."
He stressed that only trained and authorized operators should handle forklifts, and that no worker should be transported on them. Workers working at height must receive adequate training and fall prevention measures.
And this is the part most people miss... Whistleblowing reports are vital in preventing incidents and holding companies accountable. Mr. Tan highlighted the importance of public awareness and encouraged workers and citizens to report unsafe practices.
"MOM will act swiftly on such reports to prevent lapses from escalating into incidents. We thank the whistleblower in this case, who may have saved a life," he added.
As we approach the festive season, Mr. Tan warned companies against rushing to meet deadlines at the expense of workplace safety. "No deadline or commercial pressure justifies compromising safety. MOM will hold errant companies and individuals accountable," he asserted.
The consequences of unsafe practices can be severe. In February 2024, a warehouse assistant operating a forklift struck a 66-year-old man in his left leg, resulting in amputation below the knee. The forklift operator was sentenced to 20 days' jail.
In another tragic incident in October 2021, a forklift stacker operator's negligence led to the death of a 49-year-old quality control surveyor. The operator was sentenced to 138 days' jail.
These cases serve as a stark reminder of the potential consequences of unsafe practices.
